Welcome to Unity Health System’s 2009 Online Open Enrollment

To begin making your elections, click on the Choose Benefits tab and click on each benefit link to view the available options.  Refer to the Online Enrollment Guide for step-by-step instructions, which can be found in your Open Enrollment packet or by clicking here. 

Helpful information and reference documents can be found by clicking on the plan links (e.g. Blue Point Basic) on the election pages.

Your deadline to enroll in and/or change your benefit elections is November 7, 2008.  All benefit changes will take effect January 1, 2009.  If you complete your enrollment online, do not return a paper enrollment form to Human Resources.

For benefit related questions, call the Benefits Hotline at 368-3211; your call will be returned promptly.  For online enrollment assistance, call 368-3204.

Refer to your paper enrollment form for benefit costs; only costs for health and dental coverage appear online due to system limitations.  Elections made online are pending until reviewed by Human Resources; an official confirmation will be mailed to your home near the end of November 2008.
